By completing and submitting this form, I consent to GuildSuper using my TFN to search the ATO’s database to locate any other super accounts held on my behalf.  I understand that:

• If any ATO*-held lost super is found, it will automatically be transferred over to my GuildSuper account, and

• If any accounts within other superannuation funds are found, I will be notified of the results so that I may decide whether or not to consolidate those monies into GuildSuper.

I understand that this consent is ongoing until I change it by contacting the Fund.

* ATO held money includes unclaimed super, low income super contributions, superannuation holding accounts, government contributions and super guarantee contributions that are in your name.
