| Member Personal Contributions To make a personal contribution complete this Member Personal Contribution form and follow the payment instructions. |
| Member Change of Details If you change your name, address or any other important detail, complete the Member change of details form. |
| Rollover Form To roll another super account into GuildSuper, complete the Rollover Form. |
| Investment Alteration To change your investment selection, complete the Change of Investment Details form. |
| Additional Insurance To apply for Additional Insurance, complete this form. Please note this form is not required for Automatic Death Only insurance. |
| Direct Debit Request To have contributions taken out of your bank account, complete this Direct Debit Request. |

| Direct Debit Cancellation Request To cancel your direct debit payments, complete this Direct Debit Cancellation Request. |

| Employee Choice Standard Choice Form This form can be completed by employees when choosing GuildSuper as their super fund for contributions. |
| Superannuation Contribution Splitting Form To request your superannuation contributions to be split into your spouse's account, complete the Superannuation Contributions Splitting form. |
| Complying Fund Letter This letter should be supplied by an employee when choosing GuildSuper under the new Choice of Fund legislation. |
| GuildSuper Annual Report 2007/2008 This Annual Report is design to inform members of the events that occured during the year which affect the operations, management or financial condition of GuildSuper. |
