Medical Requirements (from 1 July 2009)
When applying for additional insurance you may be required to provide additional medical requirements.
Additional Death and TPD Information Requirements
In all cases of Additional insurance cover regardless of the sum insured, a Personal Underwriting Statement Part 1 (included in the Product Disclosure Statement (PDS)) is required to be completed. If you are seeking insurance cover in excess of $1,000,000, or have answered YES to any of the questions 4 to 9 of the Personal Underwriting Statement Part 1, you will also need to complete the Personal Underwriting Statement Part 2 (included in the PDS), and undergo additional tests as indicated in the table below.

*Note: Personal Medical Attendants' Reports may be requested on applications over $3,000,000 and on a discretionary basis. Additional information may also be requested depending upon the contents of the Personal Underwriting Statements. Cover limit includes any Automatic insurance cover you may have. ECG is an electrocardiogram (a test of the electrical activity of the heart, performed by your doctor). Blood tests include HIV, MBA 20 and Hepatitis B & C.
Additional Income Protection Information Requirements
In all cases, regardless of the amount insured, a Personal Underwriting Statement Part 1 and Part 2 (included in the PDS) must be completed. Further information may also be required depending on the amount of insurance cover you have selected. Please see the table below for further information.
